when to send in the contingency form

This is going to be the first time I send in the contingency form to CCS.  I was wondering when it's due.  Do I fill it out before the season or every race weekend?  I looked at the form and in the rulebook but couldn't see any such info.

You need to have one filled out every weekend and give it to the tech guys when the tech your bike.
