It's your form, do what you want with them. :)  We do this every year to make it easier for others to use.  I know I hate printing, filling them out by hand, and then re-scanning or faxing it in.  With this I can fill it out on my computer and fax it directly from the computer.

Ok, I have added the fields for track name and date and uploaded the correct form.  As well as a few other forms.

Silly question but does CCS allow u to put in the name u go by or does it have to be your legal name .example I go by tipsy hensley .can I do that or do I have to fill out my real name.  Silly I know just no one knows me by my real name

Legal name, if there is room in name field I can add nickname in quotes.

thanks gixxer 188. CCS should put those on their website. Prob make their life easier as well as ours so they dont have to try to read peoples handwriting
