Anyone figure out the procedure on how to sign up for a CCS License and an ASRA without an existing AMA membership?  Where's the option box to check for the additional $19 fee for the AMA membership (as explained in the second section of the mailer) in order to get a combined 2013 CCS/ASRA license??
