racing resume help?

Hello all,
I'm trying to put together a racing resume to get some sponsorship for next season.
Only problem I'm a total newb and have no idea where to start.   
Any pointers or exaples would be very gratly appreciated.

Don't waste your time or the person you are sending it to by making it too long or wordy. I would suggest one page listing your past accomplishments and future goals

*Vital statistics/info
  Name
  Parents (if you are a minor)
  Age/ Date of Birth
  Address
  Primary number and time you can be reached
  Alternate or Cell phone number
  Fax number
  Email address
  Web address if applicable
  What do you do for a living (student, job, etc.)

*Background info;
  Years of racing / type of racing
  Circuits raced / classes raced
  Achievements; races (with dates and locations), finishes, podiums, etc.
  Sponsors (who, how long, etc.)
  Plate Number
  Racing references (factory race directors, circuit race directors, etc. Please include contact info)
  Race plans (what are your intentions this season, next year? What are your goals?)
  Other racing experience (motocross, watercross, etc.)
  What you do in the off season to get ready for racing (weight training, etc.)

*MISC
  Photos are a great tool if you are seeking sponsorships but are not necessary if your goal is class advancement. If seeking sponsorship pictures that include trophies, your bike or anything else that might make a positive impression are a good idea.
