It's your form, do what you want with them. :) We do this every year to make it easier for others to use. I know I hate printing, filling them out by hand, and then re-scanning or faxing it in. With this I can fill it out on my computer and fax it directly from the computer.

I see that I missed the track name and date on the top of the pre-entry form. I'll fix it later tonight and upload a new version. Sorry about that.

Silly question but does CCS allow u to put in the name u go by or does it have to be your legal name .example I go by tipsy hensley .can I do that or do I have to fill out my real name. Silly I know just no one knows me by my real name
Legal name, if there is room in name field I can add nickname in quotes.
