Title: Racer/Crew Poll- Trackside Physical Medicine Services Inquiry

Good day,

I am checking out the feasibility of extending what I do for our team and a few other racers out to the rest of the racing paddock; riders, crew, support people...everyone but spectators. I am checking to see what you, the racer, would first want, and then pay to have physical medicine services available to you right in the paddock. Services can include orthopedic examinations, chiropractic manipulative services, sports taping/kinesiotaping, stretching, therapeutic ultrasound, sport specific massage, myofascial release, topical application, etc. Any pharmecuetical needs will be referred to the proper professionals.

Please note, for these services in a clinical setting, exams can run up to $140, manipulation alone can run anywhere from $60-70 and the ancillary stuff can run about $15-50 per service.

One of the goals is to allow for continuity of care. Records would be maintained and the racers in a series would be able to see the same health care professionals each round, as opposed to the volunteers that normally staff medical centers at the tracks. Not that there is anything wrong with the current staffing of medical centers, its just that from week to week, you may end up seeing different people of different backgrounds; EMT/RN's/LPN's?DC's/DO's/MD's. Long range goals are to have a member of the team even go with any off-site medical transports with the racer/crew health care file so ER personel have complete medical information available and there is a contact at the hospital for the team or family.

What about specific physical repairs, Doc.

While I've heard a lot of riders complaining about carpal tunnel issues, I think there's been a great increase in cerebral rectal inversions around the paddock. 

Would you have the opportunity to offer different levels of care?  I don't know if a cranial rectalotomy can be performed at the med shack in Illinois due to some issues, but Wisconsin might be a good opportunity.  Could be performed after the race day is over?  Maybe at a location in Wisconsin?

It can have some inflaming properties depending upon personal pressure, family history, etc.

I have been doing some kinesiotaping for arm pump issues.  RE: carpal tunnel, therapeutic ultrasound would be tried first, to be conservative, but if need be, a referral out is possible. I'd like to have an orthopedic surgeon that I could utilize for the surgical or pharmecuetical cases.

I forgot to mention, this is primarily for AMA events.  But the privateer is important too.  We were looking at two options, pay for services ala cart, or a one-time "up front fee" for "unlimited" care throughout the entire season.

I think Super Dave is hoping I get a psychologist onstaff too!

Thanks for focusing this into the AMA arena though.  I think that market is going to be better to develop something like this with the budgets of riding families seemingly increasing to meet the demands of today's racing costs. 

Have you talked to Kaiser about it?  There might be issues with insurance and all that might play a bit of havoc on things.  The idea has merit.

It would be a cash only thing. I would give a receipt to the rider/patient that would denote the diagnosis and CPT codes for the services rendered.  With how racers are and how it may be seeing a patient only once or twice and with the distance, it would have to be cash with the patient then submitting the receipt to their carrier to be reimbersed. I would not be in thier "PPO" or "HMO" plan for the most part anyway.

Any tips are appreciated too!
