Title: does anyone have Yamaha mfg. contingency info??

The link on the CCS site takes you to a Yamaha site but is nothing but retail sales of stuff, no racing info that i can see. https://www.yamaha-motor.com/404.htm?aspxerrorpath=/raceresults/contingencyform_rr.aspx
Need the info for Yamaha mfg. contingency payout forms. I know i filled some kind of form out early this year, but you know how paperwork gets lost sometimes..
